DHCT is the provider of choice for many social care employers in Cornwall supporting recruitment and training for care workers who may work in residential or nursing homes, domiciliary care, day centres or some clinical healthcare settings.
This course is for learners employed as a lead practitioner in adult care, e.g. community care officer, physiotherapy assistant, re-ablement support worker, social care assistant or telecare assistant. A lead practitioner will guide and inspire team members to make positive differences to someones life when they are faced with physical, practical, social, emotional or intellectual challenges
You must hold a level 3 qualification in health and social care or equivalent
You will complete the following mandatory modules:

- Understand Safeguarding and Protection in Care Settings
- Person-centred Assessment and Planning to Improve Individual Outcomes

- Professional Practice in Adult Care Develop,
- Maintain and Use Records and Reports
- Partnership Working in Care Settings
- Safeguard Children and Young People Who Are Present in Care Settings
- + sector specific modules related to your workplace
The duration for this course is 12 months maximum, but you can complete earlier.
